We provide a reasonably priced version of our own dental savings plan to assist our patients without insurance. Our Ballwin Dental Savings Plan runs on a yearly basis Your membership will automatically renew annually on the anniversary of your original application, and you will be automatically charged, unless you terminate your participation in the Plan. If you cancel your membership within the first thirty days after enrollment, or within thirty days after the automatic renewal on the anniversary of your original application, you will receive a reimbursement of the annual fee paid. If you have received discounted services and terminate, you will be billed for the full price of the services provided. Any refund will be paid within thirty days of the termination of your Plan, but may be reduced by any discounted services provided prior to termination. If you do not wish to continue your participation in the Plan, please notify our billing department immediately.

Limitations

• Plan fees and services are subject to change. You will be notified in writing of any changes in the Plan fees and options prior to renewal.
• Plans do not entitle you to discounts from providers in other offices.
• To enroll, you must be current in all of your obligations to Ballwin Dental Care.
• Plans are not retroactive, and the discounts only apply for services rendered after enrollment.
• It is your responsibility to utilize the applicable Plan services within the applicable Plan year. Any unused services will not be carried over into a subsequent Plan year.
• Plan fees are not refundable. Except for cancellation during the first thirty days after enrollment or within thirty days after the automatic renewal on the anniversary of your original application.
• Please be sure to notify Ballwin Dental Care of any change in your credit card or other billing information. If your enrollment is terminated for failure to pay, any unused services will be forfeited. We will take reasonable steps to notify you of non-payment.
• The Plan options are not transferable.
• You are responsible for payment of all additional services at the time services are provided. If you fail to pay at the time services are provided, any discounts will be forfeited.
• We understand that other individual dental plans can be expensive and may not give you everything you want or need. Our In-Office Discount Plan (the “Plan”) provides a significant savings for you with additional benefits!
• The Plan is not insurance.
• The Plan provides discounts for certain dental services in this office only.
• Each Plan Member is obligated to pay for all dental care services but will receive a discount for certain dental services.
